I am wanting to carryout a questionnaire with the players and parents regarding training and matches. 
 Does anyone have any examples or templates they would be willing to share?

Hi, 
 I did one around Christmas and got some interesting feedback, won't let me attach but I asked... 
 As your coach I am open to feedback on how you are finding training and match games, would you mind just letting me know how you are finding things and what you think as a team we can build on. 
 Do you think the training sessions help our match day performance? Yes/No 
 Do you enjoy training sessions? Yes/No 
 What do you like or dislike about training sessions? 
 What would you like us to work on more in training? 
 Any training exercises you want us to try as a team?
