Hi.

After completing the introduction to first aid which I thought was very well constructed, and given events over the past 12 months,  I looked for a standard medical format which my local club might adopt to gather relevant medical information e.g. Myocardial family history / Allergy's etc. I have not so far been able to find one. Any help or pointers appreciated.
